Appointments & Refills

A centralized call center in our business office is dedicated to helping with scheduling appointments, taking prescription refills and answering medical questions for both the Troy and Piqua office.  Please call 937-339-5355 for all medical services. 

Appointments, prescriptions refills, and messages can be sent on-line via our secure patient portal, My UVFC.  Patients requesting this service are provided with a personalized login and password.

Call 911 if your situation requires emergency medical attention.

Appointments

Automated appointment confirmation calls are made 24 to 48 hours in advance of appointments.  Please follow the prompts and notify us if you cannot make an appointment.  Cancellation messages can also be left on our phone system 24 hours a day by calling 937-339-5355, option 2.  Upper Valley Family Care reserves the right to charge for no show appointments and dismiss patients for repeated no shows.

Refills

A voice mailbox has been provided to allow patients to leave refill requests 24 hours a day.  Call 937-339-5355, option 3.  Refills can also be requested on-line through My UVFC or by calling the pharmacy. 

Refill requests are handled during regular office hours Monday through Friday.  Please allow at least 48 hours for processing.

We encourage everyone to check their medications before all appointments for refill needs.  If you will need a refill before your next regular appointment, please tell your nurse.

Late Arrival Policy

Running on time benefits everyone and we make every effort to see our patients on time.  Patients are seen in the order scheduled.  If you are not here at your scheduled time, we will take the next scheduled patient.  We will be glad to see you as time then permits, but we will take patients who arrive on time first.  We do reserve the right to request late arriving patients to reschedule or to see another provider if available.
