Upper Valley Family Care was started in 1981 by Craig Critchley, D.O. and Richard Plumb, D.O. in Troy, Ohio. As the practice grew, additional physicians, staff and facilities were added. James Burkhardt, D.O. joined the practice in 1986; James McNerney, D.O. joined in 1996; Shruti Fernandes, M.D. joined in 2006; and Kendall Erdahl joined in 2007. In addition to Physicians, Upper Valley Family Care has expanded the scope of services provided with the addition of Nurse Practitioners, a Physician Assistant, and a Physical Therapist.

An office in Piqua, Ohio was added in 1986. Still growing, a separate business office was leased in 1991.  A new Troy facility was constructed in 1998 in order to consolidate the Troy offices.  The Piqua office moved to larger facilities in 2009 to accomodate additional professional staff.

Today, Upper Valley Family Care has a staff of over 40 employees, each serving an important facet of patient care.
